Armidale Care Navigation Program

HealthWISE is funded by the Hunter New England Central Coast Primary Health Network to deliver a Care Navigation service for recent humanitarian entrants with Ezidi ethnicity in Armidale. 

The aim of this free service is to assist individuals and families from the Ezidi community with identifying their goals, and to problem solve solutions for barriers that may be impacting their overall health.

Examples of these may include; understanding the Australian healthcare system, navigating the health system, assistance managing appointments, referrals onto support groups or other organisations, and health literacy to manage or improve health outcomes.

The Care Navigator will meet with a client to identify what the person’s goals are and work together to achieve these. Follow up appointments will be made if necessary – this can be by phone or in person.

This is not a clinical service; instead it has been designed to work in collaboration with the General Practitioner and their patient to overcome any barriers that could be impacting on the patient achieving optimum health outcomes. This program is accessed by GP referral only, and is only available in the Armidale area.
